To obtain records form the county Health Department, all requests must be in writing and submitted in one of three ways. Records that can be scanned and emailed to you will be provided at no charge. Please include an email address in your request.

Septic System and Well Records
Septic system and well information requests are handled by our Mount Kisco office. The completed request form in the link that follows may be faxed to (914) 864-7341 or mailed to Westchester County Health Department, 118 North Bedford Road, Suite 100, Mount Kisco NY 10549. Requests to search our records for the approved plan of the well on a property can be made by completing the Sketch Request form.
When completing this form please provide as much detailed information as possible, especially, the address, the original tax map designation (Section, Block and Lot Number). Please be advised, some municipalities may give a property multiple tax map designations, and we need the original tax map designation of the lot, municipality, the year the house was built and dates of any additions (bedrooms) to house. The municipality’s tax assessor and/or the building department can assist you in securing this information. Be sure to provide your phone number so that if we have any questions we can reach you. Please note that if the records requested are in our archives, the record retrieval can take at least 10 days.
Medical Records (Children)
To obtain a copy of your or your child's Health Department medical records, by law, you must submit a signed HIPPA release.
We will acknowledge your request in writing and will inform you of the approximate time it will take to prepare your records. You will receive a FOIL Request Number that you'll need to reference in all future correspondence with us.

Fees
There is a 25-cent, per-page fee for copying most records, but there is no charge to e-mail records. Copies of medical records cost 75 cents per page. You also have the right to review the records, by appointment, and can then request copies be made for you. Depending on the volume of copies you request and our workflow, copies may be mailed to you at a later date, if they cannot be provided to you at that time.
Once the records are ready, if copies cost less than $5, the Health Department will send them to you and request payment. If copies cost $5 or more, the Health Department will inform you of the cost, and payment must be received in the Records Access Office before any copies of the records are sent.
Please note the FOIL Request Number on the face of your check so we can track your payment and locate the records you've requested. You will also be asked to provide the FOIL Request Number in any telephone inquiries you make.
